| Minimum Qualifications | *Permanent Transfer Candidates: Current NYS employee with one year of permanent or contingent permanent service as a Community Health Program Manager 1 (CHPM1), M2 OR Current Department of Health employee with permanent or permanent-contingent service as a Community Health Program Manager 2 (CHPM2), M3 OR current DOH or other NYS employee with one year of permanent or contingent-permanent service in a G-27 or M3 or higher title and eligible for transfer under section 52.6 of the Civil Service Law to Community Health Program Manager 2 OR Current NYS employee with one year of permanent or contingent-permanent service in a G-27 or M2 or higher title and eligible for transfer under section 70.4 of Civil Service Law by virtue of possessing the minimum qualifications below OR **Provisional Candidates: any individual meeting these minimum qualifications: A Bachelor's or higher degree AND professional administrative work experience in a governmental agency's community health/preventive health program, including program management and supervision of professional staff, as follows: six years of general administrative experience, three of which must have included second level supervision and program management. (Program management responsibilities must have involved oversight of the design and/or implementation of public health outreach and prevention or chronic disease or communicable disease surveillance or control programs focusing on the delivery of public health services to the general public. This experience must include five or more of the following responsibilities: program planning; program implementation; program evaluation; program monitoring; preparation of grant requests or grant administration; development of policy and/or procedures to implement policy; contract management; budgeting and fiscal management; staffing and staff development; inter-governmental communications and relations. Experience limited to administrative support of public health programs such as personnel, budgeting or health care finance is not considered qualifying, nor is the provision of medical services to a fixed population. |
| Preferred Qualifications | Master's degree in Public Health, Public Administration or a related field; four or more years of professional work experience related to the management of a large bureau or other large governmental organization with statewide programs; three years of professional work experience in public health, epidemiology or a related field; a strong understanding of state and medical professional processes, including quality improvement, evaluation, policy development, communication, contract management, legislative processes; and excellent organizational and communications skills. |
| Responsibilities | The Community Health Program Manager 2 will serve as the Bureau of Immunization's Director of Quality Improvement and Policy and provide overall coordination, management and monitoring of the Bureau of Immunization's quality improvement, evaluation activities and policy development, including improvement of service delivery systems, provision of vaccine, provider practices, staff improvement, school immunization rates, and implementation of special initiatives to improve immunization rates in NYS. The incumbent will oversee the Bureau's quality improvement, evaluation and policy development activities related to vaccine provision, relevant information technology development, Bureau communication activities, provider and school quality improvement activities and participate in the establishment and implementation of immunization related legislation and regulations. This position will represent the Bureau at state and national conferences and in interactions with the federal government, state agencies, medical providers, schools, and other stakeholders. |
| Conditions of Employment | *Full time, permanent appointment for those eligible for transfer. **Provisional, pending examination for all others. Travel, within and outside of NYS, to locations that may not be served by public transportation is required. |
