Filing a Foodborne Illness Complaint

All foodborne illness complaints are investigated by local health departments. If you suspect the food you ate made you ill, please contact your doctor and your local health department or the local State District Office. Contact information can be found by using the Department's interactive map.

Filing a General Complaint about a Food Service Establishment

Complaints are investigated by the county health department or State District Office responsible for the area where the restaurant is located. To file a complaint about a restaurant in New York State, you can contact the county health department or State District Office. Filing a Complaint about a Grocery Store, Convenience Store or Other Similar Food Store

Contact your local health department or State District Office if you believe that food you purchased at a grocery store made you ill (see "Filing a Foodborne Illness Complaint" above).

If you did not become ill, but wish to file a complaint about a grocery store in New York State, contact the New York State Department of Agriculture and Markets. The Department of Agriculture and Markets licenses and inspects grocery stores, convenience stores and other similar food stores in New York State.
