1. How do I create an account to access the Public Authenticated NYSE-CON system?

A training manual is available to assist with the steps necessary. Follow the steps below to access the manual:

  1. Go to this link http://www.health.ny.gov/facilities/cons/nysecon/
  2. In the Public Authenticated NYSE-CON System section click on the Training link
  3. Click on the link titled Printable Account Creation and Login Guide
  4. Follow the steps detailed in the document to create an account. Please note that if one has a valid HCS account it can be used to access Public Authenticated NYSE-CON as well. The suggestion is still to continue using the NYSE-CON link that can be found in the URL listed in step 1.

2. What training information is available for the Public Authenticated NYSE-CON system?

There are self paced learning modules as well as training documentation available for users. Follow the steps below to access the training:

  1. Go to this link http://www.health.ny.gov/facilities/cons/nysecon/
  2. In the Public Authenticated NYSE-CON System section click on the Training link
  3. The self paced learning modules and training documentation will be available on this page.

3. Where can I get the necessary CON schedules to submit with my application?

The latest schedules are available online for one to download and complete. The Public Authenticated NYSE-CON system will indicate which schedules one needs to submit based on the type of application being submitted. Follow the steps below to access the schedules:

  1. Go to this link http://www.health.ny.gov/facilities/cons/nysecon/
  2. Under the CON Applications bullet click on the Forms and Schedules link

4. How can I get rid of the pop up message asking if I would like to access a non-trusted site while navigating in Public View?

If you are using Internet Explorer 7 or 8, follow the following steps:

  1. Select the Tools/Internet Options and then the Security tab
  2. Select the Trusted Sites zone
  3. Select the Sites button
  4. Enter https://apps.health.ny.gov in the trusted sites field and select the Add button
  5. Close the first window and then select OK to return to the web page

5. How do I add multiple instances of a document in the Correspondence section?

When Schedule 2 is selected as the document type you will see the following message 'A unique description is required for this document.', please use the following unique information in the description in this order: Last Name, First Name Schedule # section letter

For example:

  1. Smith, Pat Schedule 2
  2. Jones, Pat Schedule 2a
  3. Smith, Ann Schedule 2b
  4. Smith, John Schedule 2c

This will create a separate instance that can be updated individually as needed throughout the process.

6. How do I reset my password?

  1. Select Forgot Password on the Public Account Management System on the left of the login page.
  2. You will have to answer the 3 security questions that you had entered when the ID was first created.

Note: If you do not know the answers to the security questions you must create a new User ID.

7. How do I get access to my project(s) if I can no longer log in?

  1. Create a new User ID
  2. Send an email to nysecon@health.state.ny.us requesting access to your project. Please include the following information: facility name, submission #, and contact information for the new user.
  3. You will be contacted if additional information is required.
