This is a procurement which encompasses 3 components. In order to apply for any of the components, eligible applicants must submit separate applications for each Component on the New York State Grants Gateway.

Component A: Retention in Care Support for HIV+ Clients Receiving Co-located Medical Care at Drug Treatment Programs in New York City
Component B: Acute Medical Care, Disease Screening and Linkage to Medical Homes for Clients of Authorized Syringe Exchange Programs Located Outside of New York City
Component C: Enhanced Outreach and Linkage to Care for Active Substance Users Who Currently are Not in Drug Treatment Located Outside of New York City

Applicants should submit their applications, at a minimum, one (1) hour prior to the submission deadline. The system will perform an application error check and all identified issues must be resolved before the application is successfully submitted. Failure to leave adequate time to address issues identified during this process may jeopardize an applicant's ability to submit their application. The Grants Gateway will notify applicants of successful submission.
