Adolescent Preventive Care-Screening Questionnaires

The New York State Department of Health / Division of Managed Care and Program Evaluation (DMCPE) has introduced measures of preventive care for adolescents. The measures are based on recommendations of national organizations such as American Medical Association's Guidelines for Adolescent Preventive Services (AMA GAPS) and the American Academy of Pediatrics. During the past few years of work on this subject we have measured the quality of care for adolescents with various methodologies including record review and self-reported experience with care. Both methodologies have determined that using a standardized checklist or screening tool increases the delivery of the recommended preventive care.

To help MCOs and providers with adopting the use of tools, we have the following examples for use:

NYS DOH Instrument - developed through a collaborative initiative with plans and experts in adolescent medicine. A trigger questionnaire is available as well as a provider information component to facilitate use of the instrument.

American Medical Association (AMA) GAPS Questionnaires - two versions are available, one for younger adolescents and one for older adolescents. Tools may be used in their current form. If more than 5 questions are changed, AMA has requested attribution as adopted from AMA GAPS.
