Information for Prescribers - Mandatory Generic Drug Program

  • Prescribers, or an agent of the prescriber, must call the prior authorization line at 1-877-309-9493 and respond to a series of questions that identify the prescriber, the patient and the reason for prescribing this drug. The Mandatory Generic Program Prescriber Worksheet and Instructions (PDF, 39KB, 2pg) provide step-by-step assistance in completing the prior authorization process.
  • At the end of the call, when all questions are answered, an eleven-digit prior authorization number will be issued. The prior authorization number must be written on the face of the prescription. The prescriber must write "DAW and Brand Medically Necessary" on the face of the prescription.
  • For orders written by an unlicensed intern or resident, the supervising physician's MMIS ID number or their license number must be entered. Hospital, clinic, or other health care facility MMIS ID numbers cannot be used.
  • The completed prescription may be filled at any NYS Medicaid enrolled pharmacy that stocks the drug.
  • The toll free prior authorization call line can also be used to cancel a prior authorization number that was obtained as long as the drug was not dispensed.
