Health Homes Learning Collaborative
In the near future, the Department of Health will launch a Health Homes Learning Collaborative to provide a forum for identifying and sharing best practices in Health Home design and implementation. Approved Phase I and II Provider-Led Health Homes will be invited to apply to participate in this Learning Collaborative opportunity. Findings that emerge from this forum will help guide ongoing implementation efforts and inform state policy decisions around this significant delivery system reform effort.
The Health Homes Learning Collaborative aims to build from the success of the earlier CIDP Learning Collaborative, which identified critical elements of successful care management programs for high-need beneficiaries, and in doing so, helped shape New York's current Health Home strategy. A summary of findings from the CIDP Learning Collaborative is available below, which may be helpful in informing Health Home implementation efforts.
Approximately 12-15 Provider-led Health Homes will be selected to participate in the Health Homes Learning Collaborative, including a representative mix from across the state. All other approved providers will have access to emerging findings from the Learning Collaborative through an ongoing webinar series. The Learning Collaborative will be run by the Center for Health Care Strategies, in close partnership with DOH and with support from the NYS Health Foundation.
