Implementation of Health Homes : HIT Adoption Training Webinars
To enhance the Health Information Technology component of the Health Home program, which will serve as a priority care coordination model for Medicaid in NY State, the NYS Department of Health, Office of Health Information Technology Transformation (OHITT), in collaboration with the two Regional Extension Centers in New York, have put together a series of webinars based on their experience in bringing priority primary care providers to Meaningful Use.
The webinars will focus on tools and workflows for adoption of electronic health records (EHRs). Because of the similarity in adoption and support services needed for Health Homes, this series of webinars will provide assistance to Health Homes and their partners to enhance the implementation of EHRs within the Health Home Network.

January 24, 2012: Selecting an Electronic Health Record
This webinar will provide participants with guidance on the key steps for effective electronic health record (EHR) vendor selection. The session will help prioritize software requirements, educate the vendor to identify features and functional components necessary to meet the clinical needs of the organization.

March 7, 2012: Optimizing the Practice Workflow
This training session will provide guidance and best practices to participants on how to engage clinicians and office staff in workflow redesign when adopting health information technology. The tools referenced in this session provide a framework to support the documentation of current workflows, identification of staff roles and responsibilities and how to think about the changes technology will introduce in to the day-to-day activities of a practice.

Webinar participants will learn how the goals of Patient-Centered Medical Homes (PCMH) and Health Homes (HH) are aligned and how the Electronic Health Record supports both. A crosswalk with PCMH and Meaningful Use measures will further emphasize alignment of goals. Explanations of the supporting evidence required to achieve PCMH recognition and examples will be given. Additional resources for further information and technical assistance will be provided. This session will provide participants with information on NCQA Patient Centered Medical Home (PCMH) and Meaningful Use (MU) . Presenters will describe each initiative, discuss the overlap in measures between the two and provide guidance on how to achieve recognition and incentives from the programs.
