DSRIP Application Prototype

The Prototype DSRIP Application provides a hypothetical response to the DSRIP Organizational Application and a selection of the DSRIP Project Applications, to serve as an example to PPSs. The Prototype Application is not intended to be a model response and there are areas where it could have been improved, which are discussed in the Independent Assessor's scoring and responses to the Prototype Application below. The Companion Guide provides further insight and commentary on the Prototype DSRIP Application, to support PPSs in understanding the Application and the priorities and concerns of the Independent Assessor.

Scale and Speed Template

The Scale and Speed template for DSRIP Project Plan Applications contains only the speed and scale elements of the Project Plan Applications. PPSs should complete and return this template to their DSRIP Support Team (DST) contact from KPMG no later than December 16th, although they may submit before that date. DST contacts will work with PPSs to help identify any errors or inconsistencies in their scale and speed figures prior to the Project Plan Application submission deadline on December 22nd. Speed and scale figures will drive around 40% of the total valuation and errors could have large unintended consequences for overall application score.

A number of PPSs have had questions about the definitions of provider types in the Project Scale tables. Clarification about exactly which providers/entities will be included in each category in these tables will be provided in a separate document.

NY DSRIP PPS Plan Application and Scoring Guide - Overview

The NY Draft DSRIP PPS Plan Application materials are now available for public review and comment. The state is required to engage the public and all relevant stakeholders (including the Centers for Medicare and Medicaid Services (CMS), community stakeholders, Medicaid beneficiaries, physician groups, hospitals, and health plans) by publishing the NY Draft DSRIP PPS Plan Application and Scoring Guide, including all relevant background material, and providing a public comment period that will be no less than 30 days. The process must ensure the submission of comments through electronic means. The material below includes four key artifacts that are important to the PPS application process and Independent Assessor evaluation process.

  1. DSRIP PPS Organizational Application (PDF)– The Organizational Application includes sections that summarize the PPS's organizational structure, community health issues, and capabilities to implement a successful PPS. The PPS will be required to submit an executive summary, a description of the governance structure, a description of the community needs assessment, the workforce strategy, data sharing and rapid cycle evaluation approaches, the cultural competency and health literacy strategies, DSRIP budget and flow of funds, financial sustainability of the PPS, bonus points, and the attestation of the application. This section represents 30% of the overall project application score.
  2. DSRIP Project Plan Applications (Domain 2, 3, & 4) (PDF)– Each PPS will submit no fewer than 5 projects and no more than 11 projects. The Draft DSRIP Project Plan Application outlines the project specific requirements of the PPS and the necessary data input for scoring purposes. The PPS will have the choice to select projects based on the following logic for each domain:

    • a. Domain 2 - All DSRIP plans must include at least two of the following projects based on their community needs assessment. At least one of those projects must be from sub-list A and one of these projects must be from sub-list B or C. Performing Provider Systems (PPS) can submit up to 4 projects from Domain 2 (A, B, and C). If the PPS selects 10 projects across Domain 2, 3, and 4 they are eligible to submit an 11th project for patient activation of the uninsured and low Medicaid utilizers (Project 2.d.i).
    • b. Domain 3 - All DSRIP plans must include at least two projects from this domain, based on their community needs assessment. At least one of those projects must be a behavioral health project from sub-list A. Performing Provider Systems can submit up to 4 projects from Domain 3.
    • c. Domain 4 - All DSRIP plans must include at least one project from this domain, based on their community needs assessment. Performing Provider Systems can submit up to 2 projects from Domain 4.
    Domain 1 DSRIP Project Requirements Milestones and Metrics (draft 9/29/14) (PDF)- This document outlines the Domain 1 milestones and metrics for each Domain 2 and 3 project. Specifically, the document identifies the corresponding metric/deliverable needed to be completed to demonstrate achievement of the requirement, as well as the documentation that the Independent Assessor will require to verify/validate the project requirements have been successfully achieved. For Domain 4 projects only, each PPS will submit specific milestones for Domain 4 Projects through the submission of a detailed Implementation Plan, which will be submitted by April 1, 2015. The format and content of the detailed Implementation Plan will be articulated by the Independent Assessor.

  3. DSRIP PPS Application Scoring Guide (PDF)– The scoring guide tool provides an overview of how the applications will be scored by the Independent Assessor. The guide provides an overview of the process and an example of how one project for one PPS will be scored. The guide also identifies the bonus point process.

Additionally, the DSRIP PPS Lead & Financial Stability Test will be released on October 1, 2014 to the prospective PPS entities. The NY DOH goal is to fully vet the fiscal health of the PPS entity before the application review process starts on December 16, 2014.

NY Draft DSRIP PPS Plan Application and Scoring Guide - Public Comments The state is looking to engage the public and all affected stakeholders (including community stakeholders, Medicaid beneficiaries, physician groups, hospitals, and health plans) by publishing the development of the NY Draft DSRIP PPS Plan Application and materials for no less than 30 days. NY DOH is looking for the submission of comments through electronic means by sending emails to dsripapp@health.ny.gov after 9/29/2014 – 10/29/14.

NY Draft DSRIP PPS Plan Application and Scoring Guide - PPS Outreach The Independent Assessor will have a number of opportunities to interact with the PPSs to discuss the Application and Assessment Process. NY DOH has scheduled the following activities to support the PPSs in the review of the application.

  • Webinar – The Independent Assessor will post a webinar imminently to discuss the contents of the NY Draft DSRIP PPS Plan Application and Scoring Guide. The webinar will be made available on the DSRIP website.
  • Conference Call – The Independent Assessor will host an operator assisted conference call in October prior to the expiration of the public comment period to discuss the Application and Scoring Guide.
  • Public Forum Meeting with PPS Leads - The Independent Assessor will hold an open meeting in October prior to the expiration of the public comment period to discuss the Application and Scoring Guide. Please note, this meeting will be limited to the prospective PPS lead agencies.
