State Health Department to Host Population Health Summit

State, National Leaders to Discuss Public Health Approaches to Improving Population Health

ALBANY, N.Y. (November 19, 2013) -- On Tuesday, December 3, 2013, State Health Commissioner Nirav R. Shah, M.D., M.P.H. will host a population health summit to bring together state and national leaders to heighten awareness about the role public health plays in improving population health. The Population Health Summit, focusing on "Making New York the Healthiest State: Achieving the Triple Aim," will be held at the New York Academy of Sciences, 250 Greenwich Street in Manhattan from 8:00 a.m. - 4:30 p.m. It will also be webcast at: http://www.health.ny.gov/events/webcasts

"The goal of the Population Health Summit is to increase awareness of public health approaches to improving population health and the role of all health system stakeholders in New York State," said Commissioner Shah. "All sectors, including health care providers, insurers, employers, and public health departments, need to work together to assure quality health for all New Yorkers. This summit will include presentations on how these diverse sectors are collaborating around the state, using the state's health improvement plan, the Prevention Agenda 2013-2017."

The Population Health Summit will delineate the roles of health and non-health sectors in improving population health and highlight successful efforts by local coalitions of county health departments, hospitals, clinics, schools, business, community organizations and philanthropy.

The summit will include five sessions:

  • 8:30 a.m. – 10:00 a.m.: A Whole System Approach to Achieving the Triple Aim – the New York State Experience;
  • 10:00 a.m.- 11:00 a.m.: A Conversation with CDC Director Dr. Thomas Frieden
  • 11:00 a.m. - 12:30 p.m.: Engaging Local Stakeholders in Partnerships to Improve Population Health: The NYS Prevention Agenda 2013-2017.
  • 1:15 p.m.- 2:00 p.m.: Legislative Panel Discussion with New York State Senator Kemp Hannon, Chair, New York State Standing Committee on Health and New York State Assembly Member Richard Gottfried, Chair, Assembly Health Committee
  • 2:00 p.m.- 4:00 p.m.: Role of Employers in Population Health: Improving Worker Health and Reducing Healthcare Costs

Closing remarks and a discussion of next steps will conclude the day's events at 4:30 p.m.
