Immunization Record Retention Schedule
for the Following Institutions

  • Child day care programs operated by local governments other than school districts and BOCES
    • 3 years after child attains age 18
  • School districts and BOCES, including pre-K, high school equivalency, adult education programs and child care programs these entities operate
    • 6 years, or 3 years after individual attains age 18, whichever is longer. These records may be requested beyond their minimum legal retention period by persons needing proof of certain immunizations for college admission. The State Archives recommends that school districts and BOCES evaluate the need to retain immunization records longer than the stated minimum retention period for these purposes.
  • Public community colleges, the Fashion Institute of Technology, and the entire CUNY system
    • 6 years, or 3 years after individual attains age 18, whichever is longer
  • The SUNY system including the colleges, universities and medical centers, has not as yet developed a retention schedule for immunization records.
  • Non-public schools and private post-secondary institutions
    • The State Archives is not legally authorized to set legal minimum retention periods for the records of non-governmental entities.
  • County health departments, public health nursing services, and clinics or hospitals operated by local governments, including the New York City Health and Hospitals Corporation
    • 6 years, or 3 years after individual attains age 18, whichever is longer
