Assessment, Feedback, Incentives, and eXchange Program
The Assessment, Feedback, Incentives, and eXchange (AFIX) program is a quality improvement strategy to raise immunization coverage levels and improve standards of practice at the provider level.
AFIX assessments are conducted at private and public health care provider sites. During an AFIX visit, immunization records of all active two-year-old patients are assessed as well as immunization delivery practices. Immunization records can usually be extracted from data in the New York State Immunization Information System (NYSIIS). Feedback is given to providers and their staff members with recommendations for strategies to improve immunization service delivery.
Assessments are performed on the two-year-old (24 - 35 months) patient population. Immunization records are assessed for completion of: 4 doses of DTP/DTaP (diphtheria, tetanus, pertussis/diphtheria, tetanus, acellular pertussis), 3 doses of polio, 1 dose of MMR (measles, mumps, and rubella), 3 doses of Hib (haemophilus influenza type b), 3 doses of hepatitis B, 1 dose of varicella (4:3:1:3:3:1) and 4 doses of pneumococcal conjugate vaccine (4:3:1:3:3:1:4) by the age of 24 months.
AFIX site visits are done by the local health departments (LHDs) with assistance from the regional offices as indicated. Visits are endorsed by the Centers for Disease Control and Prevention (CDC).

A series of four CoCASA training webinars were presented in 2009. The first session (Training 101) reviews the basic functions of CoCASA. Training 201 demonstrates the use and interpretation of the numerous reports in CoCASA. Training 401 and 501 have been combined into one session which demonstrates database maintenance, custom queries and reports. There is not a Training 301.
