What Are the New York State Vaccines for Children (Outside NYC)/Child Health Plus Vaccine Programs?

Publicly Purchased Vaccine is available free of charge to providers that participate in Vaccines for Chidren (VFC)/Child Health Plus B (CHP B) programs through the New York State Bureau of Immunization.

A child less than 19 years of age is eligible to receive this vaccine if he or she is:

  • Medicaid/Medicaid Managed Care enrolled. The patient is enrolled in Medicaid or a Medicaid Managed Care plan.
  • Not insured. The patient does not have any health insurance.
  • American Indian/Alaskan Native. The patient is American Indian/Alaskan Native. American Indian/Alaskan Native is defined as any individual who (1) is a member of a tribe, band, or other group of Indians including those tribes, bands, or groups terminated since 1940 and those recognized now or in the future by the State in which they reside, or who is a descendant, in the first or second degree, of any such member, or (2) is an Eskimo or Aleut or other Alaskan Native or (3) is considered by the Secretary of the Interior to be an Indian for any purpose.
  • Underinsured. The patient is underinsured. Children who have health insurance that does not include vaccines are underinsured. Children with health insurance that reimburses for immunizations are not eligible even if the insurance company requires a high deductible, co-payment, or covers only part of the cost of vaccine
  • Child Health Plus (CHPlus) enrolled. The patient is enrolled in Child Health Plus.

There is no out of pocket expense for a provider to order vaccine for eligible children.

How Do I Register?

If you wish to participate in this program and order vaccine, complete the Provider Profile and Registration Forms and Vaccine Storage and Handling Plan and return them to the NYS Bureau of Immunization, Vaccine Program, ESP Corning Tower, Room 649, Albany NY 12237. Don't forget to sign both the form and plan.

To complete the registration process, a "PIN" number will be assigned to your practice using the information supplied in the above form and plan. Once a pin is assigned, a VFC regional representative will make arrangements with your practice to set up a site visit. Until the site visit is completed, vaccine ordering will not be allowed.

When I Register, What Am I Agreeing To Do?

  • To screen for a child's eligibility.
  • To retain a record of the eligibility screening.
  • To comply with the recommended immunization schedule.
  • To update your provider profile annually.
  • To provide a current list of providers using VFC/CHPlus vaccine at your site.
  • To provide access to records if requested by the NYS Department of Health Bureau of Immunization to verify eligibility.
  • To immunize eligible children at no cost to them. (Administration fees not to exceed $17.85 may be charged as long as the service is not denied because the fee cannot be paid.)
  • To submit required accountability reports for doses administered and inventory monthly.
  • To participate in a VFC site visit and assessment of your practice's immunization levels including any follow-up visits.
