Diabetes
  1. Establish a collaborative group of diabetes experts to coordinate inpatient care of diabetes.
  2. Identify gaps in existing programs and services, and then recommend interventions.
  3. Develop standardized protocols and procedures for the management of diabetic patient care, starting with hospital care through discharge, and into the community.
  4. Expand access to diabetes support services in Saratoga County.
