  Falls prevention
  1. Design and develop health education materials to prevent falls in the elderly.
  2. Incorporate these materials into a community outreach plan. This plan will be community-focused, with participation from all hospitals, Suffolk County Health Department, and community agencies and organizations.
  3. Activities between the partners will be coordinated by the Injury Prevention group.
  4. In order to target high risk areas and subpopulations, develop Data Subcommittee that will analyze pertinent Suffolk County data regarding demographics and high risk areas.
