How to Make a Freedom of Information Law (FOIL) Request

If you wish to access records under the Freedom of Information Law (FOIL), all requests must be made in writing, whether made via mail, fax, or e-mail. You must include the following information:

  • The requestor's name, address, phone number and email address so that DOH can transmit records electronically, if practicable
  • Specific details regarding the records being requested. Please include as much detail about the records as possible, including relevant dates, names, descriptions, addresses, complaint numbers, etc.

In-person inspection and copying is available Monday through Friday between the hours of 9:00 a.m. to 5:00 p.m. Appointments are necessary and can be made by calling (518) 474-8734.
