- Serum Cholersterol and Blood Pressure and Heart Attack, Angina or Stroke (NYS Behavioral Risk Risk Factor Surveillance System-BRFSS)
- All data relates to adults 18 years of age or older. The BRFSS is a telephone based survey that collects information on self-reported health related behaviors. The specific indicators in this section are:
- Percent of respondents who reported they had their serum cholesterol checked in the past 5 years
- Percent of respondents who reported they had ever been diagnosed by a medical professional with high blood pressure.
- Percent of respondents who reported they ever had a heart attack, angina or stroke.

- Mortality (Bureau of Biometrics and Health Statistics)
- Information on deaths is extracted from death certificates. Each death receives an ICD-10 code indicating the primary reason for the death. The codes used for the rates in this section are as follows:
- Cardiovascular ICD-10(I00-I99)
- Cerebrovascular disease ICD-10(I60-I69)
- Disease of the heart ICD-10(I00-I09,I11,I13,I20-I51)
The mortality rates are rates per 100,000 population. Both crude and adjusted rates are presented.

- Hospitalization Rates (Statewide Planning and Research Cooperative System-SPARCS)
- Data on hospitalizations are collected through the hospital inpatient discharge data system, SPARCS. Data are not available on events that did not result in a hospitalization, such as cases that were only treated in a hospital emergency room. Hospitalization rates in this section are based on prinicipal diagnosis codes, which indicate the primary reason for each hospitalization. Principal diagnosis codes use the ICD-9 system to define cases and are as follows:
- Cardiovascular disease ICD-9(390-459)
- Cerebrovascular disease (Stroke) ICD-9(430-438)
- Coronary heart disease ICD-9(402, 410-414, 429)
- Hypertension ICD-9(401.0, 401.9, 402.0, 402.1, 402.9, 403.0, 403.1, 403.9, 404.0, 404.1, 404.9)
- For hypertension:
- Excludes Stage I - IV Kidney Disease with dialysis as identified by the following ICD-9 procedure codes:
- ICD-9(403.0, 403.1, 403.9, 404.0, 404.1, 404.9, 389.5, 392.7, 392.9, 394.2, 394.3, 399.3, 399.4.)
- Excludes Cardiac Procedures as identified by the following ICD-9 procedure codes:
