Data notes:
a Statewide Planning and Research Cooperative System-SPARCS - Data on hospitalization are collected through the hospital inpatient discharge data system. Each hospitalization receives an ICD-9 code at discharge which indicates the primary reason for the hospitalization. Data are not available on events that did not result in a hospitalization, such as cases that were only treated in a hospital emergency room.
b Vital Records: Information is extracted from birth, death and fetal death certificates. When percentages are calculated records with missing information for the indicator of interest are excluded from the calculation. Population based rates use population estimates provided by the US Census Bureau. ICD-10 codes are used for cause of death.
c Expanded BRFSS: All data relates to adults 18 years of age or older. The Expanded BRFSS was a telephone-based survey that collects information on self-reported health-related behaviors. It was conducted between July 2008 and June 2009 in 57 NYC counties excluding NYC. Rates for NYC counties are from the 2008 NYCDOHMH Community Health Survey.
