Reporting Data Completeness Issues

The 2011 Inpatient Annual Report was completed using data reported as of 10/3/2012. The completeness of the SPARCS data is based on two reviews: the SPARCS Annual Reconciliation Process and a comparison to the Institutional Cost Report (ICR) as supplied by the facilities to the Department of Health. At the conclusion of the 2011 SPARCS Annual Reconciliation Process, the following inpatient facilities reported two or more months below their expected target of monthly discharges.

As of 10/3/2012, the 2011 total inpatient discharges collected by SPARCS were 98% of the total discharges reported on the ICR. This indicates that the 2011 SPARCS inpatient reporting is complete.

Adirondack Medical Center at Lake Placid 0306 Facility reported its emergency department data under the Adirondack Medical Center at Saranac Lake, PFI 0324, due to the small number of patients. Effective 1/1/2012 Adirondack Medical Center at Lake Placid will begin reporting the emergency department data under its PFI 0324.
Ellenville Regional 1002 A critical care hospital, provides inpatient services, but the facility was unable to submit SPARCS data due to a technical issue. The facility will begin reporting 6/1/2012.
McClellan Campus 0830 Facility does not provide inpatient services. The facility continues to retain inpatient services on its operating certificate.
Monroe Community Hospital 0414 A rehabilitation hospital with few inpatient beds. Inpatient patient records are generally less than ten per year.
North General Hospital 2968 Closed 07/09/2011.
Orange Regional Medical Center 0699 Opened a new facility at 707 East Main Street Middletown, New York closing the older hospitals in Goshen, New York and at 60 Prospect Avenue Middletown, New York.
Oswego Hospital Alvin L Krakau 0012 Reported its inpatient and outpatient services data under Oswego Hospital, PFI 0727. Effective 1/1/2012 Oswego Hospital Alvin L Krakau will begin reporting the inpatient and outpatient services data under its own PFI.
Rockefeller University Hospital 1465 A research hospital with no charged inpatient beds.
Sisters of Charity Hospital 0218 Facility had fewer records than expected by September, 2012. By January, 2013 the facility finished reporting the proper number of patient records.
St. Francis Hospital Beacon Division 0179 Closed 5/1/2009, but has continued to report services under this PFI. The facility is in the process of removing these reports to the St. Francis Hospital Poughkeepsie Division, PFI 0180.
St. Josephs Hospital of Cheektowaga 0292 Facility had fewer records than expected by September, 2012. By January, 2013 the facility finished reporting the proper number of patient records.
St. Luke's Cornwall Hospital 0698 Closed 07/01/2011.
SVCMC St. Vincent's Manhattan Hospital 1471 Closed 08/05/2011.
