Appendix TT-Abortion Record Editing

Physician license numbers are a non-identifying but restricted field, only released on records which do not contain any indications of abortion. On those so indicated records, all physician license numbers would be blanked. Full release of physician license numbers present on abortion indicated records requires specific DPRB approval, as in this case the physician license number is considered to be an identifying field.

Physician license numbers are released according to the following guidelines, which result in the setting of the Abortion Flag, Inpatient Data Element Number 139 and Outpatient Data Element Number 105.

  1. Physician license numbers are releasable on all records where there is no indication of an induced abortion on the input record.

    Abortion Flag = N

  2. When there is an indication of induced abortion, all physician license numbers are to be blanked for records identified by interrogating all diagnosis and procedure fields on the record for the following ICD codes or CPT-4/HCPCS codes.

    All Diagnosis code fields to include: 635-639 (includes all codes with 4th and 5th digits; i.e. 635.00, 639.92, 637.4, etc.) or 779.6

    All ICD-9 Procedure code fields to include: 69.0 (69.00 - 69.09), 69.5 (69.50-69.59), 69.6 (69.60-69.69), 69.93, 73.8 (73.80-73.89), 74.91, 75.0 (75.00-75.09), or 96.49

    All CPT-4/HCPCS codes to include 01964, 59812, 59820, 59821, 59830, 59840, 59841, 59850, 59851, 59852, 59855, 59856, 59857, 59866, 59100, 59200, 88304, 88305, S 8055, S0190, S0191, S0199, S2260, S2262, S2265, S2266 or S2267

    Abortion Flag = Y
