Definition:

A composite field comprised of portions of the patient last name, first name, and social security number. This field, in conjunction with the Patient Sex and the Patient Birth Date is designed to provide probabilistic matching criteria for individual patient records in longitudinal analyses without compromising the confidentiality of the record.

Included below are the individual components of this data element.

  • "First 2" and "Last 2" characters of the Patient's Last Name.  The birth name of the patient is preferable if it is available on the facility's information system.

  • "First 2" characters of the Patient's First Name

  • "Last 4" digits of the Patient's Social Security Number.

NOTE:  This data element is NOT to be confused with the Patient Control Number, which provides linkage of all record types containing patient-related data for a specific discharge.
