Stage II - Immunization Reporting Core Measure

Eligible Professionals and Eligible Hospitals in New York City

The Citywide Immunization Registry (CIR) accepts electronic data from Eligible Providers and Eligible Hospitals located in New York City.

Objective

Capability to submit electronic data to immunization registries or immunization information systems except where prohibited, and in accordance with applicable law and practice.

Measure

Successful ongoing submission of electronic immunization data from CEHRT to an immunization registry or immunization information system for the entire EHR reporting period.

Exclusions

Any EP/EH that meets one or more of the following criteria may be excluded from this objective:

1. The EP/EH does not administer any of the immunizations to any of the populations for which data is collected by their jurisdiction's immunization registry or immunization information system during the EHR reporting period;

2. The EP/EH operates in a jurisdiction for which no immunization registry or immunization information system is capable of accepting the specific standards required for CEHRT at the start of their EHR reporting period; (Note: this exclusion is not applicable to providers/hospitals located in NYC)

3. The EP/EH operates in a jurisdiction where no immunization registry or immunization information system provides information timely on capability to receive immunization data; or

4. The EP/EH operates in a jurisdiction for which no immunization registry or immunization information system that is capable of accepting the specific standards required by CEHRT at the start of their EHR reporting period can enroll additional Eligible Professionals. (Note: this exclusion is not currently applicable to providers/hospitals located in NYC)

Providers/hospitals immunizing only adults (19 and above) will need to obtain patient consent (verbal or written) before reporting to the CIR. Please visit the CIR FAQs for more information.

Meeting the Stage 2 Immunization Reporting Measure in New York City

Registration of Intent

Eligible Professionals and Eligible Hospitals wishing to meet the Stage 2 public health measures in New York must register their intent to submit data to the NYS Department of Health and/or the NYC Department of Health and Mental Hygiene.

Data Specifications

Eligible Professionals and Eligible Hospitals in NYC must use the CIR HL7 specifications when submitting data to the CIR. The specifications document can be found on the Information for EHR vendors page on the CIR website.

If you currently submit immunization information in HL7 format to the CIR:

Continue submitting as usual using the CIR's HL7 web service. As acknowledgement of ongoing submission, please retain an ACK message returned by the CIR to your EMR application from a submission made during the reporting period.

Eligible Professionals and Eligible Hospitals that met the immunization reporting requirement in Stage 1 using HL7 v. 2.3.1 and became ongoing submitters can continue to submit data in HL7 v. 2.3.1 and still satisfy the reporting requirement for Stage 2.

If you do not currently submit immunization information in HL7 format to the CIR:

If you will be reporting to the CIR in HL7 format for the first time, you must engage in an onboarding process with the CIR. After you have registered your intent to submit immunization data to the CIR, please follow these steps:

1. Ensure that you are registered with the CIR and have a valid CIR facility code.

2. Sign and return a copy of the CIR's healthcare provider confidentiality statement.

3. Obtain test account credentials from CIR staff.

4. Work with your EHR vendor to setup an HL7 2.5.1 web service interface connection to the CIR's test environment and complete the testing process

5. When testing has been completed, take the interface 'live' by moving submissions to the CIR's production web service

6. Log HL7 acknowledgement (ACK) messages that are sent by the CIR's HL7 web service in response to each transaction sent by the provider's EHR. These ACK messages will serve as evidence that ongoing data transmission was achieved

Data Transport

Eligible Hospitals and Eligible Providers should submit HL7 data to the CIR using the CIR's HL7 real-time web service. For more information please visit the CIR's Meaningful Use page.
