Stage II - Syndromic Surveillance Reporting Core Measure

The New York City Department of Health and Mental Hygiene accepts electronic syndromic surveillance data from Eligible Hospitals physically located in New York City.


Capability to submit electronic syndromic surveillance data to public health agencies, except where prohibited, and in accordance with applicable law and practice.


Successful ongoing submission of electronic syndromic surveillance data from Certified EHR Technology to a public health agency for the entire EHR reporting period.


New York City has the capability to receive data electronically from Eligible Hospitals; only EHs that don't have an emergency or urgent care department are excluded.

Meeting the Stage 2 Syndromic Surveillance Reporting Measures in New York City

These guidelines are for Eligible Hospitals physically located in New York City. For New York State requirements for those outside of New York City, please see the NYS DOH Syndromic Surveillance information page.


Eligible Hospitals and Critical Access Hospitals wishing to meet the Stage 2 public health measures in New York City must register their intent to submit data to NYS DOH. Please see the NYS DOH registration instructions.

The New York City HL7 Specifications for Syndromic Surveillance Stage 2 submissions require HL7 2.5.1 and closely follows the PHINS Messaging Guide.

Process Summary

NYC DOHMH Syndromic IT staff will provide you with implementation materials and will be available to support you if you have questions during the process. At a high level, the steps are:

  1. Following Registration of Intent, NYC DOHMH will request additional contact information from your implementation team
  2. NYC DOHMH will assist in setting up UPHN Lite for data transport, if this has not already been arranged.
  3. NYC DOHMH will request various test files from your EHR vendor or IT contacts to check for correct HL7 2.5.1 formatting.
  4. NYC DOHMH will request files to verify test data correctness, completeness and consistency with specifications from the EHR system. For any inconsistencies, NYC DOHMH will request EHR vendor/IT contacts to troubleshoot and resolve. For eligible hospitals currently sending NYC DOHMH Syndromic Surveillance data in alternative formats, side by side testing comparing with the MU specifications be performed.
  5. When NYC DOHMH has verified that test messages meet reporting requirements, ongoing submission will be instituted in accordance with NYC DOHMH standards.
