Single Source Procurement: Single Source Procurement: One-stop STD Clinic Enhancement in Support of ETE Blueprint Recommendations

Pursuant to New York State Finance Law § 163.10(b), The New York State Department of Health is presenting the following summary of relevant circumstances, and material and substantial reasons why a competitive procurement was not feasible.

The NYSDOH AIDS Institute has executed a contract amendment to C030592 to the existing joint venture with the New York City Department of Health and Mental Hygiene (NYCDOHMH), which supports the implementation of a one-stop-shop model of comprehensive services within the eight NYCDOHMH Sexually Transmitted Disease (STD) Clinics located throughout the five boroughs. The additional funds will support expansion of HIV testing services in NYCDOHMH clinics and increased access to testing, provision of partner services and information/referral for HIV prevention and care to clinic patients. Additionally, funds will support the coordination of all grant-related activities to ensure all programmatic components are occurring in a timely manner and as per NYSDOH requirements.

The goal of this contract is to establish the one-stop-shop model, which ensures prompt access to medical care for persons engaging in high-risk sexual and drug using behaviors. This model addresses gaps and improves upon prevention and support services being provided to persons engaging in high-risk behaviors (e.g., men who have sex with men, injection drug users, commercial sex workers, etc.)

These additional funds to the contract are aimed at continuing support for the established NYCDOHMH one-stop-shop model. This model allows for the provision of a continuum of medical care services to both HIV-positive and HIV-negative individuals including HIV and STD treatment; prevention services, both bio-medical and behavioral; and supportive care services such as referrals to housing, transportation, and food. The ultimate goals of this model is to provide patient-centered services to HIV-negative persons engaging in the highest risk behaviors and HIV-positive person to prevent the acquisition and transmission of HIV, and to support the Governor's Plan to End the Epidemic by 2020. Through this service model the provision of interventions and services seek to:

  • Intensify high impact prevention efforts in communities where HIV is most heavily concentrated;
  • Expand targeted efforts to prevent HIV infection using a combination of effective, evidence-based approaches;
  • Integrate STD and HIV testing and treatment, referrals and education within eight NYC STD clinics;
  • Increase HIV testing and partner services;
  • Develop a robust Pre-Exposure Prophylaxis (PrEP) educational and referral system within NYC and surrounding counties;
  • Increase linkage and retention in care for STD clinic patients newly diagnosed with HIV, previously diagnosed patients that were never linked to care, and patients disengaged in HIV medical care lacking continuity;
  • Hire and train HIV prevention specialists (navigators) to link HIV-negative STD clinic patients to PrEP programs and optimize access to prevention and supportive services (medication adherence, insurance and prescription assistance program, transportation, housing, etc.); and address patient barriers to linkage and retention in HIV medical care (entitlements assistance, transportation, housing/behavioral health needs etc.) for HIV-positive persons; and
  • Train/re-train STD clinic medical staff on antiretroviral medications, updated clinical guidelines, and the provision quality HIV medical care.

The expected outcomes are to increase efficiency with which people at risk or who are living with HIV are linked to and retained in HIV treatment, prevention, and support services.

NYCDOHMH is and continues to be the epi-center of the HIV epidemic within NYS. The NYCDOHMH is the local health authority for the five boroughs of NYC and they are the only provider operating NYC's STD clinics.
