Money Follows the Person Demonstration Part A and Part B

Pursuant to New York State Finance Law § 163.10(b), The New York State Department of Health is presenting the following summary of relevant circumstances, and material and substantial reasons why a competitive procurement was not feasible. The New York State Department of Health Money Follows the Person Demonstration Part A and Part B Program has made a determination to issue a Single Source Procurement to one eligible grantees to support ongoing currently funded projects.

As part of the Money Follows the Person (MFP) program, New York State currently contracts with the New York Association on Independent Living (NYAIL) to administer community transition efforts through the Open Doors Transition Centers. The purpose of this program is to re-balance Medicaid long-term care spending by identifying potential participants in nursing facilities who are interested in transitioning to the community, educating them on their return to community options, and facilitating successful transitions to their community of choice.

The work of the Transition Centers facilitates community transitions that otherwise could not have occurred or would have resulted in reinstitutionalization. Transition Specialists funded by MFP provide crucial support during the transition period. They serve as a “bridge” between discharge planning in the nursing facility and the start of HCBS in the community. Peers provide one-on-one support to individuals and families interested in transitioning to community living. By sharing their personal experiences in living with a disability or long-term health care needs, they promote successful transitions through community integration. Nursing home Outreach Educators support the development of a culture of ‘transition readiness’ among facility staff, facilitating referrals to the Local Contact Agency (NYAIL) and collaboration with the Open Doors program.
