Letters of Interest
The following list of entities indicated an interest in submitting a Health Home Application to Serve Individuals with Intellectual and/or Developmental Disabilities and provided additional information requested by the State on September 14, 2017. Letters of Interest are non-binding and entities that did not submit a Letter of Interest or respond to the September 14, 2017 letter are not precluded from submitting a Health Home Application to Serve Individuals with Intellectual and/or Developmental Disabilities. Applications are due on November 30, 2017 and Applicants will be required to meet the minimum standards outlined in Part II Section A of the CCO/HH Application.
