Conclusion:
Provider unsuccessfully appealed the Independent Assessor´s findings relative to the Governance Milestone 1 for DSRIP Year 1, Quarter 2.

Independent Assessor initially failed the PPS for Governance Milestone 1 which required the PPS to demonstrate that the governing body and subcommittees have been successfully and formally established. For this milestone, the PPS failed to submit the Organizational Chart, Meeting Schedule Template and Governance Committee Template during the initial submission for the DSRIP Year 1, Quarter 2 report. Upon remediation, the PPS submitted the equivalent of two of the three requirements. The Governance Committee Template remained outstanding.

Upon appeal, the PPS submitted a Governance Committee Template as attached to the appeal. However, the IA did not receive the Governance Committee Template timely and in a manner described in the protocols. As a result, the Independent Assessor upheld its original determination on appeal.
