Q4 Innovation Executive Summary. Please describe the innovation, and how it addresses the social determinants of health. Please identify how the innovation addresses the 6 innovation criteria (i.e. ROI, scalability, feasibility, evidence–based support for innovation, relevance to the Medicaid population and speed to market).

Grand Manor Nursing & Rehabilitation Center has initiated a Telemedicine Pilot Program to better respond to their resident´s ever changing physical condition that necessitate the need for an immediate assessment and actions to be taken by a physician. The main goal of this DSRP funded Telemedicine pilot is to increase quality of care for the residents through timely physician consultation visits via telemedicine.

Changes in a resident´s physical condition can necessitate the need for an immediate assessment and additional actions to be taken by a physician. The payor source for the majority of Grand Manor residents are Medicaid and Medicare.

This pilot program allows for physicians to consult with the resident, via the telemedicine system avoiding unnecessary transfer to the hospital. A nurse participating in the telemedicine session at the patient site can assist in providing the physician additional information that may be lacking when conducting a visit in their office and or clinic setting.

Amongst the many non–medical benefits are the ability using the same HIPAA compliant telehealth platform to connect distant family members, residents and caregivers, to develop more formidable care plans, and conduct interventions during resident mood and other significant changes.

Additional applications of the system are for resident admission, MDS (minimum data set) requirements, care planning, pulmonary and cardiac evaluations and the access to additional physician specialists not readily available within a skilled nursing facility.

The pilot program will soon expand and include St. Barnabas Hospital, Bronx, NY with the goals of reducing unnecessary post–operative surgical visits to physician offices, rehabilitation services in post acute and skilled nursing, and to improve SBH´s palliative and hospice care program.

The program also includes distance learning and training for professional and staff in working with Telehealth in LTC settings.

The Telemedicine Pilot Program was designed by The Castleton Group in collaboration with technology partners My Health Connection (telemedicine platform), MyNICAS (non–invasive hemodynamic monitoring and medication management of polypharmacy patients), Advanced Medical Devices (telemedicine carts for bedside examinations) and Wound Matrix (wound measurement and healing tablet technology).
