Q4 Innovation Executive Summary. Please describe the innovation, and how it addresses the social determinants of health. Please identify how the innovation addresses the 6 innovation criteria (i.e. ROI, scalability, feasibility, evidence–based support for innovation, relevance to the Medicaid population and speed to market).

Rapid Access Dental Clinic eventually expanding to an Urgent Care Model at Albany Memorial Hospital. Dental visits to our Emergency Departments are one of the top 20 reasons our Medicaid and uninsured patients seek services. By expanding access to dental clinic appointments, we will encourage new and existing patients to access our services in a more appropriate manner. We would also employ self–scheduling technology that would allow patients to see open slots and schedule though their smart phone. Eventually we would seek to co–locate dental services in the Albany Memorial Emergency Department which has the highest rate of dental visits in our system.

We would also begin using and SDOH screening tool to assure patients who are in need of additional support would be linked with appropriate services. Patients who are in need of a PCP would also have this linkage made with one of our PCP providers.

The ROI would be substantial as it would reduce costly and inappropriate ED utilization and increase utilization of less costly and appropriate services. This model will start out by offering expanded clinic hours in the evening 3 days per week and based on volume expand to a fulltime Urgent Care model to be co–located in one of our Emergency Departments. The American Dental Association supports several evidenced based models for a closer relationship between Ed´s and dentists. The Medicaid population has a gap in their service connection to appropriate dental services and many dentists do not accept Medicaid leading to access issues. We have applied for funds through our DSRIP participation and anticipate getting the Rapid Access clinic operational by Fall of 2018.
