Successful Integration of UAS–NY: A Two Phase Process

Transition: includes all the steps and activities that organizations must take before assessments are administered using the UAS-NY
Implementation: begins when staff start using the UAS-NY to administer assessments

Organizational Transition Activities

  • Establish a leadership team; appoint a UAS–NY Coordinator
  • Learn fundamental concepts about the UAS–NY application
  • Conduct an organizational assessment
  • Ensure staff access to UAS–NY
  • Promote staff readiness
  • Conduct UAS–NY one–time set–up activities

Statewide Implementation Plan

  • Transition to UAS–NY began March 2013 and will be fully implemented statewide by March 2014
  • UAS–NY will replace existing assessment instruments in the following programs/plans:
    • Adult Day Health Care
    • Assisted Living Program
    • Care at Home Waivers I & II
    • Consumer Directed Home Care
    • Long Term Home Health Care
    • Managed Long Term Care
    • Nursing Home Transition and Diversion Waiver
    • Personal Care
    • Traumatic Brain Injury Waiver

Statewide Implementation Plan
