Enclosed are documents updated with the current Income Eligibility Guidelines to be used in the administration of the Child and Adult Care Food Program. These items include:

Distribute the Letter to Households to participants along with the Income Eligibility Form for Child Care Centers (DOH-3688) or the Income Eligibility Form for Adult Day Care Centers (DOH-3834). Please use the enclosed order form (CACFP-3718) to obtain copies and for other languages. Forms are also available by calling 1-800-942-3858 and selecting option 5, or at our website www.health.ny.gov/CACFP - CACFP-3687
The Income Eligibility Guidelines have been updated and should be used beginning July 2018 to categorize participants as Free, Reduced or Paid. Income Eligibility Forms are not required to be collected for Head Start Programs, Homeless Shelters and At-Risk Only programs. - CACFP-3854C
Please complete the Participant Data Form for each center and keep it on file. CACFP staff will view it at the next review of your CACFP operations. For CIPS-certified, update this information online on each center application.
