Interventions:

  • Recruit and hire primary care providers
  • Hold quarterly cancer screenings in partnership with the Cancer Services Program of Oneida-Herkimer-Madison Counties
  • Add a Life Coach to the NYS Diabetes Prevention Program To increase the percentage of adults whose hemoglobin a1c levels are in good control [less than 8%]
  • Provide support and guidance to participants enrolled in 16-week program, followed by 6-8 months of monthly post-core sessions
  • Use media and health communication to build public awareness and demand for cancer screenings and self management classes
  • Collaborate with community health partners, schools, and early childcare settings to integrate 5-2-1-0 daily goals into wellness policies and practices
