  • Eliminate exposure to secondhand smoke
  • Promote culturally relevant chronic disease self-management education

Interventions:

  • Promoting of policies and practices in support of breastfeeding
  • Increasing the availability, accessibility and use of evidence-based interventions in self-care management in clinical and community settings
  • Promote use of Farmers' markets; hospital become "baby-friendly" for breastfeeding
  • Support "The Good Life Program" and "Eat Smart NY program"
  • Implement "I'm moving, I'm Learning" and/or the CATCH curriculum in classrooms
  • Develop list of free resources available to support worksite wellness efforts
  • Reduce sodium content in all meals at N-W Community Hospital
  • Expand Chronic Disease Self-Management Program "Living Healthy"

  • Support "The Good Life" (obesity) Program, Support "Eat Smart NY" Program; reduce sodium content in all N-WCH meals; train staff to talk with patients about their weight and tobacco use; ensure decision support/reminder tools of EHR are used
  • Promoting of policies and practices in support of breastfeeding
