- Provided a clear rationale for selecting Prevention Agenda priorities and included the identification of a health disparity they will be addressing
- Identified process and outcomes measures toward achieving Prevention Agenda objectives in the selected priorities
- Described how the organizations will adapt an evidence-based approach
- Provided a detailed work plan and identified their roles and the roles of their partners in the implementation of the plan
- Provided a brief plan for how they will engage partners to sustain the efforts
