  1. OCHD is meeting with Medina Memorial Health Care System, Genesee County Health Department, United Memorial Medical Center, Wyoming County Health Department and Wyoming County Community Hospital to explore tri-county needs in access to quality health care.
  2. Collaborate to apply for grants to address needs for tri-county area. A grant would support an FTE to conduct follow-up on discharges from the three hospitals to avert hospital readmissions.
  3. Locally, meet to identify all current health, social service and educational resources available to the community to determine gaps in services, and to identify where resources can be shared.
  4. The same approach will be used to improve health literacy to enhance wellness, prevention and early intervention to increase awareness of available services.

  1. Orleans County Health Department (OCHD) is meeting every third Thursday of the month with Medina Memorial Health Care System (MMHCS), Genesee County Health Department, United Memorial Medical Center, Wyoming County Health Department and Wyoming County Community Hospital to explore the tri-county needs in access to quality health care and chronic disease.
  2. Collaborate to apply for grants to MPHSP 2010-2013 MPHSP Part A
  3. Conduct follow-up on discharges from the three hospitals in order to prevent readmissions into the hospitals.
  4. Meet with local partners to identify all current health, social service and educational resources available to community to determine gaps in services, and to identify where resources can be shared.
  5. The same approach will be used to improve health literacy to enhance wellness, prevention and early intervention by promoting health literacy for chronic disease management as part of our program to address congestive heart failure and diabetes mellitus.
