Job Description

The Medical Social Worker Assistant:

  • Is involved in all aspects of coordination of care for the Children with Special Health Care Needs (CSHCN) program.
  • Develops and implements individual, comprehensive service plans for children with special health care needs.
  • Confers with professionals in public and private agencies to obtain social services for children with special needs and their families.
  • Utilizes community, medical, social and health resources to meet individual and family needs.
  • Typical work activities include counseling, advocating for and providing information to families of children with special health needs.

Minimum Qualifications:

Graduation from a regionally accredited or New York State registered four (4) year college or university with a Bachelor's Degree in psychology, sociology, social work, social welfare, or a related social work field AND one year of full-time paid social casework experience dealing with families in a recognized agency adhering to acceptable social casework standards.
