The Tompkins County Home Health Care (HHC) team consists of highly skilled nurses, therapists and support staff who are dedicated to providing quality care to patients. Opened in 1966, HHC's services include IV therapy, specialized wound care and a multi-discipline approach to patient and family centered care. It is an innovator for quality care, participating in a state IPRO demonstration project, and serving as a mentor to other Certified Home Health Agencies (CHHA) in implementing outcomes- based measures. HHC was the first CHHA in the state to support 24 hour medication infusion for pulmonary hypertension, enabling patients to stay at home and out of the hospital. Staff welcomes the cultural diversity of patients and is supportive of all persons including those with high-risk needs and challenges. We recognize them for their dedication and service.
