Katie Wheeler-Martin is nominated for her collaborative efforts to improve the health of the community as part of Prevention Agenda initiatives. The Department of Behavioral and Community Health must coordinate, communicate, and report on the Community Health Improvement Plan (CHIP) to our partners and NYSDOH. To meet this requirement, Katie took on additional responsibilities related to the development of the CHIP. As epidemiologist, Katie provided the Dutchess County specific data used in formulating CHIP goals, activities and interventions. She also led the CHIP Stakeholder Forum requiring organizational skills, attention to detail and the application of critical thinking to develop materials that facilitate effective information sharing, encourage collaboration amongst representatives from various agencies while facilitating communication and enlisting partner agency commitment to activities that meet CHIP objectives. Katie's efforts on this project are but one example of the consistent high quality outcomes of Katie's exceptional work ethic and efforts.
