The Westchester County Department of Health's outreach staff of three community health workers (CHWs), one health investigator (HI), and one intern, strive for health equity by empowering self-care through education.The outreach staff serve as liaisons between patients in the community and nurse case managers, clinic nurses, and clinicians. Outreach staff measure blood pressure and discuss risk reduction strategies where indicated, supporting chronic disease self-management, consistent with the NYS Prevention Agenda. Through knowledge of health information technology, outreach staff promotes medication adherence and critical disease control through our video daily observed therapy (VDOT) program for TB patients. Through follow up in our STD clinics, outreach staff address the needs of a vulnerable at-risk population, who may otherwise go without care services. An MPH student, Sydney Leone, has developed an orientation program for new Community Outreach Staff.
