DAL: HCBS 10-04
Subject: Money Follows the Person (MFP) Demonstration
Identification of and Outreach to Nursing Home Residents Project

Dear Administrator:

The New York State Department of Health selected agencies from around the State to implement the MFP Demonstration Identification of and Outreach to Nursing Home Residents Project. The purpose of the project is to assure that nursing home residents are provided with objective information on home and community-based services.

The Department of Health will provide selected contractors with the names of certain nursing home residents as identified by the Long Term Care Minimum Data Set (MDS). Selected contractors, using a peer-based approach, will meet with those nursing home residents and/or their legal representatives, provide objective information about home and community based services to those who are interested, and, as requested, make referrals to and work with the nursing home residents' discharge planner to facilitate the nursing home resident's transition into the community. This project will not supplant any existing responsibility that the discharge planner has for assisting a nursing home resident, as requested by the nursing home residents, with transitioning into the community.

The following agencies have been selected to implement this project:

Southern Adirondack Independent Living (Glens Falls Independent Living Center) Albany North Clinton, Essex, Franklin, Fulton, Hamilton, Montgomery, Saratoga, Warren, Washington

*NOTE: The selected agency for the New York City Region has declined the award. Implementation of this

project in the New York City Region will proceed when another agency is selected.

Agencies selected to implement this project will clearly identify themselves to nursing home personnel. Nursing Home Administrators must allow staff from agencies contracted with DOH for this project into the facility. It should be noted that nursing home residents and/or their legal representatives are under no obligation to speak with staff from agencies implementing this project. However, nursing home residents and/or their legal representatives must be provided with the opportunity to interact with project staff, should the resident and/or their legal representative so desire.

Should you have any questions, please contact Tracie Crandell, MFP Demonstration Project Director, at 518-474-6580 or txc06@health.state.ny.us.
