New York State Health Care Reform Act (HCRA)

October 11, 2005

Dear Administrator/Controller:

Re: New York Health Care Reform Acts (HCRA) of 1996 and 2000 - Extension Clinic Requirements for Public Goods Pool Reporting

As detailed in my July 20, 2005 letter, the Provider List shown on the New York State Department of Health website (www.health.state.ny.us/regulations/hcra/hcrahome.htm) has now been expanded to include the names and addresses of general hospital and comprehensive diagnostic and treatment center sponsored extension and satellite clinics.

Beginning October 1, 2005, all HCRA designated hospitals and comprehensive diagnostic and treatment centers are required, as part of their monthly Public Goods Pool reporting, to review and submit to the Office of Pool Administration, their extension clinic information.

The Extension Clinic List must be reviewed, modified, if necessary, and submitted to the Office of Pool Administration each month prior to entering the financial portion of your Public Goods Pool report. You may make changes to the Extension Clinic List as often as necessary; however, you will not be able to complete the financial portion of your Public Goods Pool report unless the Extension Clinic List has been reviewed and submitted within thirty (30) days.

To review and submit your Extension Clinic List, sign on to the Office of Pool Administration web site at: www.hcrapools.org. and follow the appropriate links. You will then login to the application using your Public Goods Pool User ID and Password. A copy of the Extension Clinic Application User Document is enclosed to assist you in reviewing and submitting your facility's Extension Clinic List.

If you have any questions or concerns regarding the above, please contact the Office of Pool Administration help desk at (315) 671-3800 Monday through Friday between the hours of 8:30 a.m. and 4:30 p.m. EST.

Sincerely,

Mark H. Van Guysling
