Single Source Procurement: Money Follow the Person Nursing Home Transition

Pursuant to New York State Finance Law § 163.10(b), The New York State Department of Health is presenting the following summary of relevant circumstances, and material and substantial reasons why a competitive procurement was not feasible. The New York State Department of Health Money Follow the Person Nursing Home Transition Program has made a determination to issue a Single Source Procurement to one eligible grantees to support ongoing currently funded projects.

The Department of Health currently has a current contract with the New York State Association on Independent Living obtained through a competitive process. Additional funds will be added to the current contract to support expanded services to address food insecurity, emergency and personal protection supplies, transition specialist recruitment and retention, and barriers to waiver enrollment, as approved by CMS, through the end of the contract period. The ability to execute these deliverables is contingent on the structure of the current program. The contract cost is funded with 100% federal MFP funds.
