Single Source Procurement: Health Homes Supportive Housing Program X-1

Pursuant to New York State Finance Law §163.10(b), the New York State Department of (DOH) is presenting the following summary of relevant circumstances and substantial reasons why a competitive procurement was not feasible.

This program is under the MRT Supportive Housing Initiative and provides housing to homeless high utilizers that are enrolled in a Health Home. This is a one-year extension for 15 of the grants from 10/1/23 to 9/30/24 and one grant will be a 9 month extension. This amendment is necessary to prevent a gap in services while this grant is competitively re-procured.
