The goal of the Healthy Capital District Initiative (HCDI) is to make a measureable and sustainable improvement in the public health of the Capital Region through a reduction of non-urgent ED visits and increased engagement with primary care physicians. To accomplish this goal, the HCDI will:
- Increase the number of adults with health care coverage.
- Increase the number of adults with a regular health provider.
- Increase the number of adults who have seen a dentist in the past year.
- Increase the proportion of cancers detected in an early stage.

- Develop systematic solutions to mitigate the trend of over-use of EDs for ambulatory care sensitive conditions.
- Encourage patients to establish medical homes for primary/preventive care and choose the appropriate levels of care for their health needs, reducing the use of emergency services for non-urgent care needs.
- Improve access to urgent care.
