The Future of Primary Care

This presentation will provide information on and examples of collaborations between community health centers and hospitals, integrating social diagnosis into the patient encounter, and the emerging role of telehealth in primary care. This Grand Rounds session is a part of the NYS Department of Health’s celebration of National Public Health Week 2018 and its theme, "Changing Our Future Together."
