DAL 17-06: Notification to Operate a Temporary Residential Services/ Respite Program
Dear Administrator:
The purpose of this letter is to inform you that the Division of Adult Care Facility and Assisted Living Surveillance has developed the Notification to Operate a Temporary Residential Services/Respite Program. This notification is consistent with the amendment to Social Services Law (SSL) 461-k which requires that no operator provide temporary residential care unless the operator has notified the Department of its intent to do so.
The Model Temporary Residential Care Addendum to the Admission/Residency Agreement is required to be provided to all residents wishing to participate in the program. The attached documents will also be available on the Department of Health's website under Adult Care Facility and Assisted Living Common Application at the following link: Adult Care Facilities.
The notification must be filled out in its entirety and submitted to the Department prior to implementation of the program in accordance with SSL 461-k. Please submit the completed notification to:
