Dear Ms. Frescatore:

Thank you for your request to extend New York´s section 1115(a) demonstration, titled "Medicaid Redesign Team" (MRT) (Project Number 11-W-00114/2). The Centers for Medicare & Medicaid Services (CMS) received New York´s extension proposal on March 5, 2021. We have completed a preliminary review of your request and have determined that the state´s submission meets the requirements for a complete extension application in accordance with 42 CFR 431.412(c).

New York´s extension application will be posted on Medicaid.gov and the comment period will last 30 days in accordance with 42 CFR 431.416(b). The state´s application will be available here.

In addition, this letter also serves to inform the state that, while CMS and New York work in partnership on the extension proposal, CMS is approving a temporary extension of the MRT demonstration through March 31, 2022. The current Special Terms and Conditions (STCs), waivers, and expenditure authorities will continue to apply during the temporary extension period, including the state´s current budget neutrality per member per month (PMPM) limits for demonstration year 23. As detailed in State Medicaid Director Letter #18-009, dated August 22, 2018, CMS expects to rebase all demonstration extensions that begin on or after January 1, 2021. Therefore, CMS will work during the temporary extension period with New York to rebase it.

For the temporary extension period, the state should continue to monitor its demonstration as stipulated in the current STCs. The state should also include the temporary extension period in its evaluation design for the next extension period, once approved. In the event that the state does not obtain an extension of MRT, it should provide an assessment detailing how it met the demonstration´s goals in its final report, as described in Section VIII, STC #8.
