Heather Warner, Health Education Coordinator, Delaware County Public Health Services

Heather has been instrumental in accomplishing the work of the New York State Prevention Agenda 2013-17 and chronic disease prevention in Delaware County, most recently by acquiring a $5,000 grant from the NYS Health Foundation for the Complete Streets and Prescription Trails project in Delaware County. Heather brought Mark Fenton, a renowned expert on designing healthy, walkable communities to Delaware County for a conference attended by local and state transportation experts, town supervisors, hospitals and the community. As a result, Complete Streets policies have been adopted by three municipalities within the county. Heather is a member of the Prescription Trails workgroup which assesses new and existing walking trails in the county to provide information for healthcare providers to develop patient tailored walking prescriptions as part of the evidence based Prescription Trails program. Heather also serves as Public Information Officer, currently managing the redesign of the DCPHS website and coordinating the agency's medical countermeasures drill for spring 2015.
