Re: New York Health Care Reform Acts of 1996 and 2000 (HCRA)

This is to inform you that the New York Health Care Reform Acts of 1996 and 2000 (HCRA) require that providers must file Public Goods Pool reports electronically via the Internet.

Hospitals and diagnostic and treatment centers are also required to verify their extension clinic lists electronically every 30 days. Additionally, hospitals are required to file Public Goods Pool 1% Statewide Assessment reports.

In order to utilize the electronic filing applications, the DOH-4264 form must be fully completed and returned to the address indicated. The Office of Pool Administration will then assign a secure electronic filing user ID and password to your organization via return mail. In addition, either the DOH-4405 Provider Election Form for Medicaid Withholding or Report of Medical Assistance Surcharge Payments for Non-Electing Provider must also be completed. Please return this completed form to the Office of Pool Administration along with the Electronic Filing User ID Application form.

The electronic filing applications are designed to be very user- friendly. User documents providing a description of each screen within the application are also available for your use. The user documentation for the electronic applications can be found on the New York State Office of Pool Administration website at www.hcrapools.org. To view and/or print the applicable documentation from the website, click on the Health Care Reform Act (HCRA) button and the Documentation and Forms button, then click on the link of the document that you wish to open.
