Goals:

  • Promote evidence-based care
  • Promote activity through promotion of a 5K challenge throughout the county for all age groups
  • Prevent and reduce occurrence of mental, emotional and behavioral disorders among youth and adults
  • Collaborate with school districts to increase and implement evidence-based programs to increase positive social development and healthy lifestyles

Interventions:

  • Promoting High Five 5 K challenge throughout county to increase physical activity school districts, YMCA's, businesses, etc
  • Assess education classes for all ages on nutrition and physical activity available in the county at the local daycare centers, school districts, libraries, YMCA's etc
  • Administer evidence-based screening programs such as SBIRT, etc
  • Collaborate with Mental Health, providers and schools to administer a tool that screens Mental Emotional health in adolescents

Goals:

  • Increase screening rates for cardiovascular diseases; diabetes; and breast, cervical and colorectal cancers, especially among populations experiencing health disparities Promote evidence-based care

  • Increase the proportion of NYS children who receive comprehensive well-child-care in accordance with AAP guidelines
  • Prevent underage drinking, non-medical use of prescription pain relievers drugs by youth, and excessive alcohol consumption by adults

Interventions:

  • Increasing the availability, accessibility and use of evidence-based interventions in self-care management in clinical and community settings for diabetic patients
  • Offer access to dental care
  • Offer facilitated enrollment
  • Increase access to quality outpatient mental health services by providing comprehensive mental health counseling at Saratoga Community Health Center
  • Increase pertussis vaccination rates by vaccinating all parents and other caregivers of infants born at Saratoga Hospital; Encourage the reduction of antibiotic use at Saratoga Hospital and in the community by developing programs, policies, and procedures aimed at reducing the use of antibiotics; Continue to include infection control education as part of mandatory staff education
  • Hand hygiene and isolation practices will be monitored; Enforce the policy of using medical devices only when necessary by performing daily nursing assessments for the necessity of devices; Ensure proper maintenance of medical devices by following maintena nce schedules and routines; and provide ongoing education programs for all clinical staff on the proper methods of insertion and maintenance of devices
